Yes, look at www.hse.gov.uk (Report an Accident) then www.riddor.gov.uk
(Report an Injury or Dangerous Occurrence) which link through to this site.
I agree that it should be secure though.

Is there some requirement to report accidents at work to the Health and
Safety Executive.
They are asking for a lot of personal details

The form is not held at a *.gov.uk domain, it does not cater for a secure
transaction etc.

it is law that makes employers having to comply.

hmmm and if we did and they refused permission then what?
