This is a request for a reference, not a subject access request. You should:
1) Make sure that the student or employee has actually applied for a post
with this firm and that they want you to disclose the information requested;
2) Get the student/employee to check the accuracy of any data you intend to
send to the prospective employer (dates, qualifications, salary, etc);
3) Record the disclosure in your disclosures log.
