Contact your the Police HQ covering your area and ask for the Data
Protection Officer. They should be able to advise you on the local
procedure/requirements.
The police do not require alot of identification data to be able to conduct
the check. The items you have listed appear to be over the top. I suspect
the form comes from the licencing panel though, rather than the police. It
may be pertinent to ask the originator of the form why they require so much
information.
