The ODPC has a notification template for councillors, which you may find
helpful

It depends what they are doing. If they are acting on Council business
within their remit as elected members they should be covered by the
Council's notification. If they are acting on behalf of their political
party, the party's notification applies. For anything else they will need
their own notification.
