We have often seen such results while screening/monitoring patients with
MEN-1.
The ionised calcium can be elevated up to three years before the increase is
reflected in the total calcium.
I would ? early hyperparathyroidism and pursue a family history for MEN-1

We have a 65 yrs old female hwo came for a thyroid biopsy(Fine needle
aspiration) (17/12). The results were:

Total Calcium 2.36 mM (RV 2.1-2.55)
Alb 38 (RV 35-50)
iCa 1.48 (RV 1.15-1.22)
21/12
Total Calcium 2.29
Alb 37
iCa 1.42
PTH was sent today and I will have the results probably next week
iCa was done using AVL (Roche)
