Well as Dr Cox in "Scrubs" once described it, "the New England Journal of who gives a damn"..... Maybe we health libraries in the UK should just dropped it from our subscriptions (how much would that be worth to them?), they might then consider their international customers a bit better. If we could encourage all medical schools to do the same too....
And why are they so fixated with IP authentication? It's my understanding that having IP authentication is one of the most efficient ways of opening up your network to hackers...
So not only are they one of the most expensive journals (for institutions) to subscribe to, they want us to put our networks at risk too.

We tried to negotiate a deal with NEJM last year but they were not prepared
to quote for the whole of the NHS in England. From the core content point
of view we do not have any spare budget this year to purchase additional
journal material.
When you look at their editoiral board and their answers to comments it is
clear they do not have a clue about the NHS
