Just to respond to the point of the term disabled visitor - contact I have had with disabled activists state that the term disabled person is preferred to person with disabilities. This is because within the social and political model of disability the person is being made disabled by society and the barriers that are constructed, they are not a person with a disability as the disability or the inability to be included is caused by the barriers put up by society not the person themselves. They are being disabled by societies barriers thus a disabled person.
Up until this point, when I met this group I had always said people with disabilities - I have a brother with severe cerebral palsy and had never thought of it in that way but now I have studied these models of disability it makes sense to me. And on speaking with him and other disabled people that is what they prefer and where I will take my lead from.
However it goes to show that different people prefer different terms and we can never get it right for everyone. The National Disability Authority suggests both terms for use however the government advice is the term disabled people.
I can go on at quite a rate about this.... sorry guys!

I can't comment on rates and management but I would suggest leaving the "carers" as it stands. The risk of people taking the mickey and bringing more carers than is needed is far smaller than the risk of upsetting people (and getting the associated bad press) by trying to nail a detailed definition down.
Also, a personal hobbyhorse of mine is the use of the term "disabled visitor". The continued use of this propagates the view of the disability before the person. I'd rather "visitors with a disability". Many might scoff at this being too "PC" but it hits at the heart of equality for all visitors and changes, in a positive way, the nature of the "contract" between museum and visitor.
My wife works with children with disability (not disabled children). The child comes first, their disability second.
Just a thought.

Oh, and while I'm here, it should be "accessible toilet" not "disabled toilet". Right, I'm getting off my hobbyhorse now as I'm a tad saddle-sore...
