We have received a query for a patient with biopsy proven myositis that was started on Prednisolone. Rheumatologist want to swap Prednisolone with Mycophenolate Mofetil. She has a past history of alcohol abuse and a raised ALT (mostly of muscle origin). They are asking about specific ALT isoforms or a hepatotoxicity marker that could be used to monitor her LFT while on Mycophenolate.
My approach would be to use pre-treatment ALT and AST as baseline and look for trends in correlation with CK level -but does anyone have any other thoughts?
