A few years ago we did some work with local oncologists and renal physicians, and found the following paper, which seems to give a very good approximation of GFR in most patients.

We have been asked by our renal physicians to provide an alternative to creatinine clearance estimations. They are particularly interested in the Cockroft Gault formula and the Modification of Diet in Renal Disease (MDRD) equation.
