We in Leeds are reviewing our procedures for amending reports.
Our interpretation of the HPC standards is that once we have released erroneous reports and they have been received either in paper of electronic form by the recipient they become part of patient records and as such cannot be altered or removed as this would destroy the audit trail.
How have colleagues a) interpreted this standard and b) translated it into laboratory policy and procedure.
We have formally requested clarification of the standard from the HPC but have, as yet, received no detailed repsonse.
