You asked for tools how to assess health policy and Implementation.
In any case I think one should have a good look into the WHO Health Report
2000, which provides the soundest macro-economic methodology to make
comparision of countries health care system perfomance.
Especially interesting are all the concepts presented, and the clear
methodological background, where you can dig as deep as you want. The
blending of "hard data" (mortality ...) available and expicit collection
of expert opinion brings home interesting insights.
Of course those rankings produced in the end are always questionable, but
introducing dimension of Responsiveness and Fairness are very very
interesting indeed to characterize health care systems..
The report can be ordered in print, downloaded in PDF, and background
informations on methods used are readily available on the WHO site
