I'll send you a sample form off-list Lisa (it's based on a BBC one), but I
wanted to flag up something about this more widely.
It's common practice to request the assignment of copyright for material
such as an oral history recording, but I am increasingly concerned about the
ethics of this. People may not realise what they are signing, and in some
cases when it was explained they have preferred it is re-worded as a single
use consent not as assignation of copyright in perpetuity. This is due to
institutions increasingly (and understandably) using recordings and material
provided in good faith for one project as an income-generating tool
longer-term by posting the image/recording for possible acquisition on a
library website afterwards. One Highland community I am working with at
present feels strongly they have had material stolen this way in the past
and I am treading carefully.
Just saying we all need to be careful about this and make sure people
understand any small print on consent forms.

Do you have a form you use to gain permission to take photos and/or video
during an outreach/ off-site activity? Ours will be with children and
vulnerable adults.
