Dear John,
SPM will only update spatial orientation information in-place (in .hdr
files or in header of .nii files). This is necessary to make things
like "Realign" or Coregister" work without writing out new images.
Voxel data is in general not altered - if new voxel data is generated,
new images will be written out under a different name.
