It seems to happen to us quite a lot. In fact I am about to
e-mail the helpdesk with one such instance.
We have tended to assume that patrons are retrieving a previous session
(e.g. via a bookmark) or have not completed the process: we have come across
patrons who thought that *submitting* their name and barcode was sufficient!
