Surely Paul's point is covered in the WHO guideline ie for DM the logical
operator is OR for fasting or 2hr limits, whereas it is AND for the other
categories.
My problem is with the (unstated) confidence levels for the various cut-
offs. Perhaps they are in the original papers. Are they the same for the
different types of sample? Then there are the suspiciously rounded cut-offs
in mg/dL!
Finally what cut-off for the fasting level do people use for abandoning the
GTT and diagnosing DM? My experience is that this is variable and probably
not evidence-based.
