I remember him saying to me some 12 years ago that the secret to working in ED was quite simple
"When you are there you are on and when you are not you are off"
We do not all practice this I am certainly on call from 12-8 am after working a 4pm-8am shift
But I know that Chris was right....

I believe No 2 is wrong. Certainly on-call is included in the total hours,
but the clock only starts when you leave the house and stops when you
return. If you are resident on-call then that counts towards the 48 hrs.
On-call at home and not being called in... doesn't. The later follows an EU
ruling on some Spanish doctors.... I think.
