Incentives
Our rather naive CEO here in NZ includes 2 words repeatedly in almost all statement
"Wellness" and "incentives"
By wellness he believes that if only people would go to their GP and get regular check ups an a few tablets they will never have to come to hospital
By "incentives" he means that of you pay GPs enough they may be more available and accessible for their patients to introduce and produce the wellness.
HE REALLY REALLY BELIEVES THIS
Sounds like the brains that run healthcare in the UK have the same idea. Primary Care has the ear of the funders at present
How about some incentives to work out of hours in the ED and pick up the remnants when the wellness runs out??
I would begin with 1.5 normal rate for all work out of hours - only paid to those who are physically present in the hospital

PS excluding the occasional tragedy the average age at death in our ED is about 83 so I am not sure how we will measure the wellness. Or will people always tend to get sick and die of some (non wellness) disease eventually

Looks to me that you may have to get serious with the education and practice
extension of Nurse Practitioners and have them functioning at Reg and Sen
Reg level in the ED.
