We long since gave up trying to transcribe the clinical details into our
computer system, and have never returned them on reports. The experiences I
have had of this in the past have ranged from the comic to the tragic! We
now deal with access to clinical info at the authorisation stage by request
form scanning (Datalog), so an image of the form is available to the
authoriser, no matter which of our three sites the form entered the system.
This works brilliantly, and I would recommend it to anyone.
We address the other problem, that of sending clinical info back to the
requester as an aide-memoire, by having a section on our request form
labelled "information you wish returned with the report". This consists of
a 25 character field marked off into individual boxes for letters - like
they use on bank direct debit forms etc. The idea is to enforce block
letters and improve handwriting, and it has some effect! Whatever the
requesting doctor puts in this field goes into our system and is printed on
the form - some GPs use it for clinical info, some for the patient's
address, some for special ID numbers (eg the DVLA number for DVLA
requests). It works well for us.
