the early church fathers were not in consensus on which creatures were
proper to which evangelists. I would quote the table of equivalences but it
doesn't copy and paste well as text; if anyone on [M-R] doesn't have web
access, please let me know off-list and I'll be glad to reformat the
information and pass it along.
The author of the page also includes longish quotes from St. Irenaeus, St.
Augustine, and St. Jerome with their arguments for their particular
identifications.
As for the date of the earliest images, a link at that page also reads:
