Guidelines for the performance of GTTs specify no smoking during the GTT
as studies have shown that smoking acutely impairs glucose tolerance.
Would permitting smokers to have their usual intake of tobacco during the
GTT give a more accurate picture of their usual glucose tolerance? Or
would doing this make an inherently poorly reproducible test even more
variable?
Does anyone actually permit smoking during GTTs in their institution? I
would also be interested in attitudes to medications. The only medications
we consider to be a contraindication to a GTT are short courses of
glucocorticoids. For patients on long-term glucocorticoids we ask them to
omit their morning dose. I would be interested to hear any differing
opinions on GTT protocols.
