When I was at my previous hospital, we started an audit on Troponin T on the cardiac readers in A&E and CCU. We basically had the problem that the test was supposed to be for supporting the diagnosis of MI in difficult patients i.e. ECG equivocal, in A&E and to assess those patients suitable for discharge in CCU. Unfortunately, because there was no unique ID system for the users it was subject to much abuse by the rest of the hospital staff who used it for all sorts of different purposes. We audited for 2 weeks, looking at number of tests carried out, purpose of test, documentation and what follow-up was done because of the result. Unfortunately, I left before all the data was accounted for, but from what I could tell, we pretty much confirmed the test was being used inappropriately and that documentation was not adequate.
