Bradley's handbook of Psychology and diabetes contains most of the validated
questionnaires related to diabetes. THis is prob not exactly what you want
but you are possibly looking for a solution when you are not clear what the
problem is.
Bradley's work ( and others) showing that knowledge does not change
behaviour. Beliefs are crucial. The 'empowerment' school, as discussed at
recent BDA conference on integrating psychology into DM care, is very likely
to be the way forward and has some base in evidence as well as feeling
intuitively correct.
Equally there is evidence about doctors problems with their beliefs in this
area!

This is a hot potato locally as I can see folk rushing to make changes in DM
care- for which there is abundant evidence now that they will fail.
