I also have a simplified epidemic model that I use to teach NetLogo (attached). Note that the epidemic transmission uses system dynamics within patches with some travel, and the behaviour transmission is ABM. The behaviour envisaged is handwashing, but it's completely generic really. People adopt behaviour in response to local incidence, personal attitude about protective behaviour, and the proportion of people around them who have adopted protective behaviour. There’s more detail about the model at http://research.criticalconnections.com.au/ABMBook/ but it's pretty buried within the 'teaching NetLogo' material. I can probably extract it fairly easily if you think the model would be useful for your purposes.
