Hi,
one of our clinicians has a patient who appears to be very insulin resistant and we were wondering if a euglycaemic clamp may be a way of assessing their insulin sensitivity. Does anyone have a protocol for this? any other suggestions for assessing their sensitivity.
The patient has had type 1 diabetes for 5 years and is an insulin pump user. The issues are that they have had 8 admissions with DKA despite being on basal insulin (Degludec) in addition to their pump. They have been an inpatient for 8 weeks on IV insulin by a fixed rate and SC insulin pump and Degludec. They have required more than 100 units of basal insulin by these 3 methods weighing only 70kg
