Hi, we had a similar request from our sepsis team about a year ago.  When we sat down and discussed the issues and cost with the sepsis team it was decided that the lab could actually provide lactate analysis in the TAT required 24/7 at a far cheaper cost by making a few changes to the current lab system.  Our lab has 2 Roche Cobas 8000 lines each with a 701 module.  Lactate was introduced on both our 701 modules (originally just on one line on the 502 module) and eye catching color coded specimen bags were distributed to the sepsis team to use when they had a lactate sample that needed processing in the 1 hr TAT from collection protocol.  The color coded bag identified the sample as needing urgent attention by our specimen reception team and is processed as an urgent sample in the lab.

Hope this helps.  Actually talking to the team and asking what they wanted was very useful as they did not realise that the lab even did lactate analysis!  It seems to be working very well as we have not had any complaints and we are meeting our TAT target on most occasions for lactate.

I have just been approached by an anaesthetist, who is enquiring about POCT lactate devices. Does anyone out there use lactate POCT? What do people recommend and are there any pitfalls I should be aware of?
