We've had a request from our ICU to measure chlorhexidine (see e-mail below) - is there a lab in the UK that can do this? Nothing on assay finder.

Many thanks for your time in discussion. To recap, I am interested in measuring chlorhexidine concentrations in blood and endotracheal aspirate. Chlorhexidine is used routinely in the ICU in accordance with international guidelines to try and decontaminate the mouth and prevent ventilator associated pneumonia (which is supported by meta-analyses of randomised controlled trials). I have recently looked at mortality data which perhaps surprisingly suggests that it may be associated with increased mortality - this is published and I have attached on the paper. My thought now is that it would be reasonable to measure chlorhexidine concentrations on the hypothesis that it is systemically absorbed. This has been demonstrated before on the intact skin of children, I have attached on this paper which also describes the measurement technique (liquid chromatography and MS). There is little else in the literature although there is a toxicology report that I have as a paper copy.
So, my question is:
- do you know of a centre that could analyse biological samples for chlorhexidine?
- what would the cost of this be?
- what are the conditions for sample transport (frozen, spun etc)?
