realistic evaluation of fines for hospital discharges: Incorporating the
history of programme evaluations in the analysis." Evaluation 17(1): 21-36.
Also the paper by Clark, A. M., et al. (2005). "A realist study of the
mechanisms of cardiac rehabilitation." J Adv Nurs 52(4): 362-371 may be
useful...
