Most medical publishers have signed up to CrossRef (CrossCheck & iThenticate), so when you submit your paper to a major publishing house your article will be check for plagiarism automatically.
JISC have prepared a review of software on behalf of Universities: http://www.jisc.ac.uk/uploaded_documents/luton.pdf
Best free one I have found is: http://etest.vbi.vt.edu/etblast3/ which checks against Medline. Interestingly I have seen suggested that Google can be used for sections (most journals are held in databases where full content isn’t crawled by external search engine, I would question its effectiveness).
