Steve,
I have stopped phoning TFT after hours, as they do not
mirror the clinical situation very closely. Patients with
life-threatening thyroid storm sometimes have less extreme
TFT that fit healthy young patients with thyrotoxicosis
and patients with a TSH above the assay limit may not be
as badly affected as patients with a TSH of 20 or 30.
Patients with Graves disease sometimes have bizarre TFT
but feel only mildly affected. My feeling is that if the
results were abnormal before then the patient has survived
this long OK and can wait till next morning and if they
are new then barring worrying clinical details (which I
almost never see) they too can wait till the morning for a
doctor to cast an eye over them to confirm the results are
clinically consistent. So no midnight door knocks for TFT!
