This sounds like the lecithin/sphingomyelin ratio we used to measure on amniotic fluid to assess foetal lung maturity. We replaced the test with Palmitic acid measurements. Varley 6th Edition 1988 Page 868 has a simple method but this is on amniotic fluid. I also recall a "shake" test, shake it and the frothier it is the more surfactant there is. A colleague in the lab reckons we used to add it to chloroform and shake but I can't recall that - CRAFT moment.

Subject: Phospholipid profile in broncheolar lavage fluid
Dear colleagues
We have just received a broncheolar lavage fluid sample on a 3 year old with suspected Pulmonary Alveolar Proteinosis. The respiratory consultant would like to get the phospholipid/surfactant profile measured on this sample (taken yesterday, has been refrigerated, approx 40 ml aliquot). I can find nothing on assay finder.
Can anyone help?
