explain to the individual what you are having to do, so that they have
the chance to re-consent if they wish.

Happy New Year. If they have withdrawn their consent then there's little
else you can do except do a report for HR infroming them of this, and
based
on the evidence and facts you have to hand.
I'd also make sure the staff member is aware that HR may take further
decisions and action without having full details as they have withdrawn
Consent to contact their GP.
