The guidelines used 45 mg/mmol as being approximately equivalent to '1+'
(300 mg/L) on a protein dipstick. This is explained in the full document.
It was not meant to be an upper limit of normal but to reflect a clinical
action limit (although in the absence of haematuria, most nephrologists are
unlikely to be very interested until excretion exceeds 1 g/day
(approximately 2+ on a dipstick or 100 mg/mmol). We still quote an upper
limit of normal of 15 mg/mmol (150 mg/day)but allude to the 45 mg/mmol in a
footnote to the report.
