I agree that it would be wise to consider carefully before acting. I would hypothesize that a comment along the lines of "concerns have been raised by XYZ that some recommendations may be biased" would lead to decreased adherence, regardless of the validity of the concerns.
Another issue is that many, if not most, guidelines have had their recommendations questioned by someone. Will we need a rating for the strength of the concern?

I wonder if my rather terse contribution might have been misinterpreted? I'm not sure that any action is necessary on the basis of what Jon said. We don't know who made this request and given the highly charged atmosphere in which discussions about 'Chronic Lyme Disease' take place and the fact that the information about investigation of guideline formulation has probably not come from an unbiased source I wonder if the right thing to do might be to contact the National Guideline Clearinghouse for comment in the first instance?
