Liddy,
I'm not suggesting you change the procedure now, but for future
reference: Jiscmail provides a survey feature which is integrated with
the mailing list (this mailing list) in that notification of the start
and the end of the survey are automatically posted to the list.
Also while it does require registration with the Jiscmail server, at
least some list members will already be registered through their use of
that server in association with other Jiscmail lists.
As a listowner, you can access the survey feature from the list
management page and then list members get to it at
http://www.jiscmail.ac.uk/cgi-bin/surveys.cgi?A=hp&LMGT1=DC-ACCESSIBILITY
Having said that, I readily admit that this isn't exactly obvious, and
for listowners who missed the announcement back in Jan 2004

(or who weren't listowners at the time!), the feature isn't exactly
well-documented in the Jiscmail help pages! Maybe I'm missing something
but I couldn't find anything in the help pages about it, and it took me
a while to remember how to access the setup - and I manage dozens of
these lists!!!
