Did you consult N. Paulus, Geschichte des Ablasses im Mittelalter (3
vols., 1922-23, reprint 2000), on almost everything regarding indulgences?

M. Tangl, Die paepstlichen Kanzleiordnungen von 1200-1500 (1894, reprint
1959) pp. 330-2 nos. CXXXVI and CXXXVII has two examples.
Various Artes dictaminis (lots of them around, not so many of them
published; M. Camargo, Ars dictaminis (Typologie des sources 60, 1991)
might help to locate some) and some of the English bishops´ registers
(many volumes published, cf. D. Smith, Guide to bishops´ registers of
England and Wales, 1981, but more have appeared since) ought to contain
examples as well.
