In "Where's the Evidence? How to Find Clinical-Effectiveness Information" (Clinical Performance and Quality Health Care 1998;6(1):44-48), Julie Glanville describes a "cascade" approach to make the most efficient use of time. Starting in MEDLINE (or Google), rather than ending there as a last step, isn't consistent with her advice on efficient searching. I tend to start my biomedical searches in PARADIGM (for infection control questions) or SUMSearch (for other questions) because of their knowledge integration and filtering - and use Google regularly for product or nonmedical questions.
