Smallish flat (usually more-or-less rectangular) objects. Look up "votive tablets" in Google for examples. In Italian these are often called "tavolette".

I have seen 20th-century examples depicting automobile accidents. The folk art in some of the modern ones is marvellous.

Finally, Gennaro Luongo, ed., _Scrivere di santi. Atti del II Convegno di studio dell'Associazione italiana per lo studio della santita', dei culti e dell'agiografia, Napoli, 22-25 ottobre 1997_ (Roma: Viella, 1998) has two articles on ex votos at a modern sanctuary dealing with typology, cultural contexts, etc.:

Bronzini, Giovanni Battista [the leading student of Pugliese ex votos], "Gli ex voto testimoni parlanti della storia di un culto" (pp. 401-18).
