Is it not a fact that there is now a legal obligation for clinical services
to reveal notes to a patient who enquires.
The only restriction is that the hospital is entitled to make a charge for
administrative purposes.
I have been searching for chapter & verse but without success

When we asked whether we could see our son's notes we were told
by the Hospital concerned that we could see them if he was less
than 18 but he would need to apply himself after that.
However some of the notes may be withheld anyway. The unofficial
way for patients to see all their notes is to visit a clinic as an
outpatient. If they are given the notes whilst waiting to see the
consultant they then rush off and photocopy them.
This should not be necessary.
