Leakage of ascitic fluid usually causes right sided
hydrothorax through peritoneopleural communication.
Injection of a dye or radiolabelled albumin into
peritoeal cavity and recovering it from pleural
aspirate confirms the existence of such leak. I have
seen two cases in our ICU where a right sided
hydrothorax in liver failure patients has been tapped
with successful resolution of a tense ascites
confirming the origin of the hydrothorax. Biochemical
analysis of ascitic and perioneal fluids would give
identical results if there is a leak.
