We use the fasting 2nd void urine (untimed) to calculate the CaE, as per Gunn & Wallace. In fact, a recommendation to send such a sample
goes out on every inappropriately high PTH result. Generally it seems to work well for discriminating FHH and PHPT. At least, the local
parathyroid surgeon seems happy!
