The Royal College of Physicians (London) is soon to publish evidence-based
clinical practice guidelines on stroke. (hopefully March 2000) In this they
make recommendations on Botox and cite references to the literature. If you
have access to Frontline there will be a piece in there when they are
published.
Here are some:
Hesse S et al (1996) Ankle muscle activity before and after Botox therapy
for lower limb extensor spasticity in chronic stroke patients Stroke 27, 455
- 460
Simpson et al (1996) Botox type A in the treatment of upper extremity
spasticity: a randomised double blind lacebo-controlled trial Neurology 46,
1306 - 1310
Sampaio C et al (1997) Botox type A for the treatment of arm and hand
spasticity in stroke patients Clinical Rehabilitation 11, 3 - 7

Subject: Botox in Rx CVA
Can anyone help me with either a reference or their own experience of
treating Spasticity in stroke victims with botulinum toxin A - in particular
to improve hand function. I have a few references regarding the treatment of
torticollis, cerebral palsy but not much on the use in stroke victims and
was wondering whether if some of the spastic muscle groups were inhibited by
Botox for a while whether we could improve the function in the hand. I have
a patient whose flexor spasm is preventing her from opposing her thumb - do
you think this would work?
Thank you
Jeannette
