Searching PubMed for --
< physician patient relations[mesh] AND outcome AND continu* >
yields 63 articles, many of the more recent ones (top of the list)
appear to be on point based on the titles. One is an article by DG
Safran from the Sep. 1998 issue of the J. of Family Practice: Linking
primary care performance to outcomes of care. This study was based at
the New England Medical Center's Health Institute and concerned
patients' trust in their physicians as a correlate of outcome of care.
Not included in the above retrieval, but netted from a similar search,
is an article by KD Bertakis of UC-Davis -- Physician practice styles
and patient outcomes: differences between family practice and general
internal medicine. Med Care. 1998 Jun;36(6):879-91.
May be what you are looking for. Hope this helps.
