We are using administrative databases to study the hospital
mortality and post-discharge mortality of inpatients whose principal
diagnosis is either chronic obstructive lung disease, or stroke (we
have specified ICD-9 codes).
In the absence of any patient-specific severity of illness data in
these databases, I am wondering if there are any co-morbidity indices
that have been validated in either of these inpatient populations.
I am aware of Charlson (validated in medical breast cancer patients),
Deyo (modification of Charlson for spinal surgery patients) and
Polanczyk (modification of Deyo for congestive heart failure
patients). Is there a similar analysis for either COPD or stroke
inpatients? The goal is to enter a co-morbidity index in
regression models as a surrogate for severity of illness.
