Perhaps a simple addition might be John Last's dictionary?
Last J, editor. A Dictionary of Epidemiology. 2 ed. Oxford: Oxford
University Press. For the International Epidemiological Association; 1988.
pp. 141p. ISBN: 0-19-505481-4
I know that it is not stricly EBHC but it comes in very useful when
teaching.
