I agree with Julian. It is quite simply impracticable to offer every
test on a 24 hour basis 365 days a year and it is our duty to
educate our colleagues both at the time and subsequently of the
appropriatess of their requests. Copper and caeruloplasmin on liver
failure at 1am? Resources, particularly staff, are often at a
premium out of hours, unnecessary testing may affect care of other
patients and to who we also have a responsibility. The job of a
Consultant in Clinical Biochemistry is to advise and on occasion
this might mean a refusal. A mature colleague should be able to
accept this advise. We should resist shroud waving (unless, of
course we are doing it ourselves
