i found barry's characterisation of the problem, and a potential solution,
helpful in sorting this out.
as you know, the cochrane is set up to permit multiple paper and
electronic publication, and has been a model for all of us.
a partial solution that we've successfully negotiated with an enlightened
publisher of our 2nd edition of the ebm book gives them just the paper
rights and we retain the electronic rights to all the supplementary
materials, CATs, CATMaker, etc so that we can give them away to folks with
limited resources (eg, developing countries) and charge the wealthy enough
to pay for the former distribution.
given our narrow focus (we're all internists), this system also gives our
electronic contributors (a dozen disciplines will contribute educational
packages, weblinks, and whatever else they want so that the beast is
relevant to other disciplines) the opportunity to expand their
contributions into full-sized books.
