Would you help my patient with whiplash injury.
Histotry is as below.
1.At 24 Apr 1997 when he was riding a motorcycle,he was knocked off by a car
from right side.At that time he was striked on the head and left lower
extremity.
Brain was intact by MRI.
He could not walk for left gonalgia for three weeks.
He put on a neck collar for 1 week.
2.At 28 July 1997 he was referred to me.
He had dull pain at the right and left base line of the neck.
He could not watch television with sitting,so he was lying when watching
television.
The treatment was only electric and hot modalities.
He had a numbness in left thumb and left big toe.
He had a pain of left eye and tears streamed involuntalily.
3.At Apr. 1998 my examination was as below.
(1)ROM test
neck flex. 25, ext.40, left flex.25, right flex 15, left rot.55,
right rot.45
(2)Tendernes
around base line of neck
(3)Motion pain
When he stretched his left base of neck(right flex.) he had strong pain.
(4)He could not sleep for left strong gonalgia.He said that it was owing
to bruise.
4.At 30 May 1998 he complained that his neck dull pain didn't disappear
and had
stretch pain at base of the neck.
His ROM of neck was not improved compared to previous time.
When I was trying isometric exercise for neck muscle,slight resistance
caused pain.
As for left gonalgia(knee pain) he could not lie on his back without hip
external rotation.
When I changed his hip in neutral position and made traction,he
complained hip pain.
When he was walking,there was no knee pain.
(Question)
1.What is the cause of hip pain?
2.How can I treat his neck?
How can I remove his dull pain and stretch pain?
