Recently we received results back from the UK for a 71 year old male with negative LGI1 and positive CASPAR2.
Previous results: anti NMDAR negative. Anti GAD-65 negative, anti TPO negative.
Paraneoplastic antibody panel (Hu,Yo,Ri,Ma:Ta,CV2,Amphiphysin) negative. No evidence of infection.
Patient had absence seizures. EEGs and MRIs showed changes consistent with these seizures.
Patient treated empirically with IV steroids and IV immunoglobulins and discharged on 50 mg prednisone per day
and anti seizure meds were stopped.
What would be the most common ongoing treatment for this patient in the UK? Immunosuppression with steroids,
intravenous immunoglobulins, plasma exchange, or other?
