Alison - I loved the report and thank you for staying involved with this - as it is tremendously helpful to all of us.
I also agree with Jane in regard to the health system having potential to mitigate social determinants - and believe that Midwifery out of all the disciplines is best positioned to tackle this. I think we are making a start with the work around Continuity of carer , POPPIE study, REACH and pregnancy circles who love to further work - I am very interested in doing a study in looking at a long term outcomes where women have received good quality (CoC etc) AN care. Feel I lack the experience to do this alone - anybody want to join me?

Completely agree, and I don't think anyone has mentioned influences on mortality and morbidity outside the health system. Michael marmots work on social determinants, Richard Wilkinson and kate pickets work on the relationship between income inequality in a country and mortality rates. The variation in maternity leave and high quality childcare are all influences on these rates. This is a brilliant starting point to really understand what the data is saying and frame questions differently.
For me, the challenge is what is the potential of a health system to mitigate the wider social determinants and how do we have greater equity in our systems?
