Dear Mariem,
you can use imcalc to set the values in your original files inside the lesion mask to "0" (assuming the the 1st image is the original image and the 2nd is the lesion mask where "1" is used for the lesion and "0" for the remaining areas:
i1.*(i2==0)
Regions with zeros or NaNs are not used for segmentation and spatial normalization so that the lesion will not influence areas outside the lesion during preprocessing.
